What is a needs assessment visit?
The needs assessment visit (NAV) is the first step in determining the optimal care plan. Our NAV is thorough and all-inclusive. We will identify the particular needs of the client. We will also examine the client’s living quarters and identify potential risk factors and advise accordingly and recommend measures to improve the safety of the premises. Further, we will discuss finances and share with the client any resources available to help fund the care expense of the loved one. We will also recommend any and all legal resources to help chart the care program moving forward into the future.
Fully understand the particular needs of the care recipient and identify those activities of daily living with which the care recipient needs help.
An assessment helps tailor care plans to the specific needs of each individual, ensuring they receive the right level of support and services.
To assess the environment of care and make any and all recommendations for changes, additional assistive devices, and other safety measures that would enhance the safety of the care recipient.
To help us assign the caregiver who is the most qualified person for that particular assignment taking into consideration client preferences, level of care required, and other compatibility factors.
To fully explain to the care recipients, family members, and/or designated representatives, the type of services we provide, the limitations to our service and the details of financial arrangements.
To identify third-party payors, if applicable (e.g. long-term care insurance), and accordingly make arrangements for client reimbursements.
To explore and identify possible sources of reimbursement for the care recipient such as state Medicaid programs, veterans benefits, etc.
To give care recipients and family members free advice on issues such as eldercare law, long-term care planning, and reverse mortgages.
Assessments can help identify ways to support the elderly in maintaining their independence for as long as possible, which is important for their overall well-being.

Our skilled live-in caregiver will assist your loved one with activities of daily living (ADL) over a 24-hour period as needed, when needed!
