As an authorized Pennsylvania Medicaid provider, Life Force delivers around-the-clock in-home support designed to fit Community HealthChoices (CHC) guidelines.
Our care model is built on CHC-approved Personal Assistance Services (PAS) hours, ensuring every visit is EVV-verified, coordinated with Service Coordinators, and aligned with each member’s care plan.

Our unique live-in caregiver service offers these outstanding benefits to CHC members, as well as to the MCOs.
Our live-in model provides a consistent caregiver who handles:
Life Force Live-In Caregivers are carefully selected, thoroughly screened, and fully trained to provide safe, professional assistance with Activities of Daily Living (ADLs, including bathing, dressing, grooming, toileting, feeding, bed/chair transfers, and ambulation.
Life Force Live-In Caregivers provide the daily structure, safety, and support that help older adults remain truly independent at home. Beyond personal care, our caregivers assist with Instrumental Activities of Daily Living (IADLs), the essential routines that keep a household running smoothly, such as meal planning and preparation, light housekeeping, laundry, shopping, medication reminders, and transportation to appointments
Life Force Live-In Caregivers help older adults stay well by maintaining the small, essential routines that protect long-term health. Caregivers provide medication reminders (not administration), encourage hydration and balanced nutrition, and offer simple exercise or mobility encouragement in accordance with each care plan. These consistent, gentle prompts reduce skipped doses, dehydration, and inactivity.
Life Force is a Pennsylvania Medicaid–enrolled provider that delivers continuous at-home coverage built around CHC-authorized PAS hours—with a special focus on night safety and the first 30 days post-discharge.
Our live-in model provides a consistent caregiver who handles:
ADLs: bathing, dressing, grooming, toileting, feeding, bed/chair transfers, ambulation.
IADLs: meal prep, light housekeeping, laundry, shopping/errands, escort to appointments.
Health routines: medication reminders (not administration), hydration/nutrition prompts, simple exercise cueing per care plan.
Supervision/cueing: for safety (e.g., bathroom escorts, redirection for dementia).
This continuity helps stabilize members at home, lowers the risk factors tied to avoidable ED use and 30-day readmissions, and reduces missed cues that happen with fragmented staffing.
Aligning to CHC requirements:
EVV-verified visits, missed-visit documentation, timely SC notifications, and clear start-of-care SLAs (receipt confirmation, rapid family contact, typical starts within 1–3 days post-authorization).
For authorizations that fund, for example, up to 16 billable PAS hours/day, we staff 24-hours and bill only the authorized hours, so members receive steady, reliable care without policy friction.
In short, Life Force gives CHC a reliable, night-strong home support option that fits your processes and keeps members safely in the community.
